If I'm doing this with the motor in the car, how can I tell if my crank is on the compression or exhaust stroke? I don't want to put everything back together and be 180 degrees out and risk bending a valve..

Why not use tie strap (plastic) to hold cam gear belt while you dress rest of belt ? And shouldn't the tensioner be released before dressing over water pump ?
Right there the entire time just need to back the crank of a half tooth while you're holding tension on the belt to the cam and crank to get the slack you needed to slip the belt on the water pump

Was this off because of the belt pulling it off? Or was it a little off from the start?

If you compress the tensioner with a 5x55mm bolt and nut it makes putting the belt on much easier
